Free Florida Injunction Forms — Approved by the Florida Supreme Court
Every form you need to ask a Florida court for protection is free. There is no charge to download them, no charge to file them, and no charge for the sheriff to serve them. This is the complete list of the official 12.980-series forms, with direct links and a plain-language description of what each one does.
The Forms Are Free
You never have to pay for a Florida injunction form
The forms used to request an injunction for protection are published by the Florida Courts and approved by the Florida Supreme Court. They are the same statewide and can be downloaded free in PDF or editable RTF format. The clerk’s office and family-law intake staff will also help you complete them at no cost.
Free forms. Free filing. Free service. Florida does not charge a filing fee for an injunction for protection against domestic, repeat, dating, or sexual violence, or stalking — and there is no cost for the sheriff to serve the respondent. §741.30(2)(a)

Petition for Injunction for Protection Against Domestic Violence
For abuse between family or household members — spouses, ex-spouses, relatives, co-parents, or people who live or have lived together as a family. §741.30

Getting & Filing the Forms
How to download, complete, and file them at no cost
- Download free. Get any form in PDF or editable RTF from flcourts.gov, or pick up paper copies from the clerk’s office.
- Complete in black ink. Type or print, give as much detail as possible, and do not sign until you are in front of a notary or deputy clerk.
- File with the clerk. In person at the courthouse in your county, or electronically through the Florida Courts E-Filing Portal. Self-represented filers are not required to e-file.
- Ask for help. Family-law intake staff and victim advocates assist with injunction paperwork for free.
